Removing an Ambulance from Service Form



Any Ambulance Service Provider who is Licensed in the State of Delaware may remove an Ambulance from service by completing this form. 

An Ambulance that is removed from service will no longer be permitted to be utilized in Delaware. 

The following is an explanation of the items required for adding, removing or replacing and ambulance application: 

  • Document is to be completed and accurate. Information must be legible, typed or written. Applications that are not legible will not be accepted. 
  • Ambulance List completed and accurate with vehicle information to be removed. 

Ambulance Service Providers are required to advise the Delaware State Fire Prevention Commission or its designated representative on any changes within the company, including, but not limited to Company contact information and Contracts for NON-Emergency Ambulance Service Providers.
